The Form 1, also known as the alpha roster, is a basic accountability sheet. It has columns that can be filled in with the names of cadets, and adjacent columns where their accountability can be marked off. It is a common sight at Noon Meal Formations where the chief clerk or squadron superintendent walks around making sure nobody is skipping formation.
In recent years, the Form 1 has controversially been used to enforce attendance at briefings and other mandatory functions by exploiting the Honor Code. After such a briefing or other event, a Form 1 is posted at the squadron CQ desk and all cadets in the squadron are required to initial it, verifying that they were at the event.
